Please use this form to sign your swimmer OUT of swim meets that they will not be attending. It is very important for coaches to have this infomation as soon as possible as the meet roster process is very time consuming. Last minute changes can cause a tremendous amount of work for the swim coach staff.
If you have children who are unavailable for different meets, please fill out a form for each child separately.
